Enter the email address on your account. We look up patient accounts by email and practitioner accounts by email (stored with your NPI). If we find a match, we email you a masked reminder of how to sign in.
SMTP must be configured on the server for reminder emails to be delivered. Password reset codes always use your Cognito user pool email.
